Hobart and William Smith Colleges are committed to ensuring our Community Standards are upheld by members of the community on and off-campus. If you have a concern that you feel impacts the well-being of the Colleges' community for you, a friend, a colleague, please review your online reporting opportunities below. You may also reach out to report concerns in-person to the Offices of Campus Safety, Campus Life, Title IX, Human Resources, or the Hobart Deans' and William Smith Deans' Offices.

Reports made in-person to the offices above or online through the links below are not confidential. In addition, while you may submit some reports anonymously, information gathered as a result of an investigation is not confidential. Confidential resources are available the Counseling Center, Office of Spiritual Engagement, and Hubbs Health Center. In addition, off-campus resources are also available, such as at Safe Harbors of the Finger Lakes.

Each report below, listed alphabetically, includes the person or office directly responsible for coordinating action and the individuals who also receive the report.


Anonymous Crime Reporting Form

Bias Incident Report

If you are aware of a crime occurring on or off-campus, or behavior that may be a violation of the Community Standards and wish to anonymously report it, please use the link below.

Submit a Report

Use this form to report an incident -verbal, written or physical-which is personally directed against or targets an individual or group based on perceived or actual characteristics such as race, color, religious belief, sex, marital status, sexual orientation, gender identity or expression, national or ethnic origin, disability, veteran status, or age; behaviors like this constitute violations of the Colleges' Community Standards. The kinds of incidents that may constitute a bias incident, include but are not limited to, threatening telephone calls or mail (including electronic mail), graffiti, physical assault, harassment, stalking, vandalism, destruction of personal property, or coercion.

Primary Coordinator of Reports: Vice President for Diversity, Equity, and Inclusion
Other receivers: Members of the Colleges' Bias Incident Reporting/Response Team (BIRT) and the Office of Campus Safety to determine if an immediate risk is present.

Campus Security Authorities

Community Assessment, Response, and Evaluation Team (CARE) report

Use this form to submit an incident which was disclosed to you and needs to be reported in your role as a Campus Safety Authority.

Submit a Report

Use this form to submit concern about a student who may be disruptive, withdrawn, tearful, declining personal hygiene, etc., or experiencing a personal challenges that may be disruptive to their ability to succeed personally or academically.

Other receivers: Members of the Colleges' CARE Team.

Hazing Reporting Form

Off-Campus Behavior Report

The purpose of this form is to notify Hobart and William Smith Colleges that a hazing incident has occurred. This form should NOT be used in an emergency. If this is an emergency and you need immediate assistance, please call the Office of Campus Safety's emergency line at 315-781-3333.

This form is utilized to report behavior regarding off-campus students, such as social events, noise concerns, upkeep concerns, and other behavior that might impact the community.

Primary Coordinator of Reports: Campus Safety
Other receivers: Associate Vice President for Campus Safety and Assistant Vice President for Campus Life; if on-file, the landlord of the property may receive a copy.

Sexual Misconduct Reporting Form

Theft of Colleges' Property

This form is an option for reporting incidents that are prohibited by the HWS Sexual Misconduct Policy: sexual assault, sexual harassment, gender based harassment, intimate partner violence, sexual exploitation, stalking, and retaliation for involvement in reporting sexual misconduct or participating in the investigation or the resolution.

Other receivers: Associate Vice President for Campus Safety (for the purposes of Clery law reporting responsibilities)

This form is utilized exclusively by Hobart and William Smith Colleges' staff to report theft of equipment owned by the Colleges.
