ORIENTATION 2015: POAP FORMS
Log in to the Orientation website to apply by completing the Pre-Orientation Adventure Program Task. The deadline is July 1, 2015. Once we have placed you in one of the program trips, you will receive a package containing information about your trip as well as a personal payment form.
You may pay by check (made out to Pre-Orientation Adventure Program) or charge the trip cost of $400 to your student account. Limited financial aid is available, so please indicate on the POAP application if you would like to be considered for a financial scholarship.

*Note: POAP requires a separate medical form than HUBBS, the campus Health Center. This medical form is exclusive to this program and to this program, only.

Please note: All trips begin on Sunday, August 23. Forms (liability waiver, equipment request form, medical form, and payment) must be on file before the program begins.
