MEDICAL FORMS
If you haven't, please log in to the Orientation Web site and download the medical information forms. You should complete and mail them to Hubbs Health Center, 119 St. Clair Street, Geneva, NY 14456-3397 as soon as possible.
If you have already mailed in your forms, do not panic if the Orientation Web site still says that you have "not completed" the medical task. Your forms are being processed, and if you are missing any information, Hubbs will contact you.
We must have all of your medical information on file by Monday, August 31, or you will not be permitted to begin classes.
