Service Catalog: Create a Survey

Web-based Survey Creation

The web-based survey tool Surveyor is being retired and replaced by a new web-based survey tool called Baseline. Surveyor will be retired on June 30, 2013 and will no longer be accessible after that date. See below for information about exporting the survey data.

New surveys should be created in Baseline. This tool may be used for both simple and complex surveys, and offers an easy-to-use interface, various question types and options, user-friendly survey URLs, and mass e-mail and reporting capabilities.

Instructions for getting started with Baseline are available on the Instructions, Training Programs, and Guides page.

Archiving Surveyor Surveys

Faculty, staff, and students who have existing surveys in Surveyor and wish to keep an archival copy of the survey data are encouraged to export the data. Instructions, both printable and video, are available.
